Tang is right, 院 is not listed as a surname. (only a courtyard)
It could be that it should be 阮 but was miswrittened.
I think it is very likely to be 阮 because it is the common Vietnamese surname: Nguyen.
However, in Mandarin 阮 is Ruan and not Yuan as you show in your photo.
I am not sure about Guangxi speaking Cantonese but both 阮 and 院 is pronounce yun in Cantonese.
Other surname characters with the Mandarin "Yuan" would be 元 from Yuan Dynasty or 袁. (both in Cantonese are pronounced Yun)
