You are so right. There are two meanings of the term ... depending on context and intonation/emphasis. One is the casual and friendly - "How goes?" And the other is the impatient and rude - "And so?!?" or "Well?!??"

And thinking about the English equivalent... "You well?" (to show concern in a casual way) versus "Well?!??" (to show impatience in a rather rude way).
