「不得了 」is a fixed expression for " seriously" 

From classical writing( 文言文)「得了」mean 「可以(結束/收拾)的」~ "something that can be (concluded /dealt with)"
in modern Chinese

「不得了」mean 「無法(結束/收拾)的」"something that can't be (concluded /dealt with)" . That imply "seriously". if you put "seriously" in all the examples listed in this thread, it would fit perfectly".

It doesn't mean translate 「不得了」 to " extremely" is wrong, it is a further implication of the original wording, just like "heroic" imply " brave" ; " seriously" imply " extremely"

佢係肥都到不得了。
He is extremely (seriously) fat.

嗰班人係蠢到不得了嘅。
That group of people is extremely (seriously) stupid.

如果那樣做, 那可不得了
if doing it like that, it would be terrible ( serious)
